Eligibility for free HPV vaccines:
The first step in protecting yourself and/or your child(ren) against HPV is completing the screener survey below. By completing this survey, our team can determine your eligibility to participate in the program. All submitted information will be kept confidential.
All eligible individuals must:
- Be between 9 and 45 years old
- Have a self-reported Texas address
- Be uninsured or underinsured
- Have not completed the HPV vaccine series before
